A progressive indigenous community is seeking a Community Care Nurse to coordinate and deliver health services to individuals and families. Qualifications

  • Bachelor of Nursing Degree from a recognized University.
  • LPN required, RN preferred.
  • Current registration with BCCNM.
  • CPR certification – C or equivalent.
  • Valid Class 5 Driver’s License and Acceptable Driver’s Abstract.
  • Acceptable Criminal Record check with Vulnerable Sector search.

About this Opportunity
Under the direction of the Health Manager, the Community Care Nurse (CCN) is responsible to coordinate and assist in the implementation and evaluation of the home and community Care, adult in-home care, discharge planning from hospital, care coordination between client & IHA, chronic disease program and various other patient care services.

The Community Care Nurse delivers and assists in the program to support people in their homes during times of acute or chronic health issues. This includes an intake process, assignment of care workers and ongoing supervision of program workers. The Community Care Nurse also provides nursing care to clients of the program such as, catheter insertion, wound care, foot care, and nursing assessment. The Community Care Nurse works with family members and community resources to ensure Elders and others with chronic health conditions are cared for safely and respectfully.

Must Haves:

  • Bachelor of Nursing Degree from a recognized University
  • LPN required RN preferred
