Position Details

Monday to Friday, Full-time Salary Employee

Join Us in Leading the Future of Health and Independence at Home

We provide care at home, delivered with heart and led by science. We are hiring an Occupational Therapist (OT) for a unique 16-week position supporting our new Hospital to Home transitional bundled home care program in the greater Ottawa area (including Renfrew and Eastern Counties). The program helps adults with higher needs reintegrate into their home and community after hospital stays, restoring independence.

Key Responsibilities
  1. Assess and implement Occupational Therapy interventions in community settings according to professional standards.
  2. Use interviews and assessment techniques to evaluate clients' needs.
  3. Provide interventions such as home safety, mobility aids, fall prevention, ADL training, cognitive strategies, and connect clients to community resources.
  4. Attend client meetings to report findings and discuss intervention plans.
  5. Coordinate care through communication with the client’s team.
  6. Document assessments, treatment plans, and progress reports.
  7. Ensure services are evidence-based and outcome-measured.
  8. Follow the code of ethics and practice standards of the College of Occupational Therapists of Ontario.
Qualifications
  1. Degree in Occupational Therapy from a recognized institution.
  2. Member in good standing with the College of Occupational Therapists of Ontario.
  3. Valid driver’s license and access to a vehicle.
  4. Experience in the field is an asset.
  5. Specialized training relevant to clinical expertise.
  6. Ability to work independently.
  7. Familiarity with outcome measurement tools.
  8. Commitment to client and family-centered care.
  9. Effective communication skills, proficient with computers and mobile devices.
  10. Registered ADP authorizer (asset).
