Hourly Rate: $37.19-46.48 (Commensurate with Experience)

Full-Time Temporary - Maternity Leave Coverage (April 2026)

Start Date – As Soon as Possible

We are seeking a dedicated Co-Director of Care (RPN)/Resident and Family Services Coordinator to support our Director of Care and clinical team in ensuring high-quality, resident-centered clinical care in our long-term care facility. This leadership role involves mentoring nursing staff, overseeing care programs, supporting infection prevention initiatives, and ensuring compliance with all regulatory standards. Additionally, this position is responsible for facilitating smooth resident admissions and ongoing placements.

Key Responsibilities:

  • Oversee and coordinate key home-wide programs
  • Skin and Wound care
  • Behavioural Supports Ontario programming
  • Infection Prevention and Control
  • CQI/Risk Management
  • Pain and Palliative Care
  • Staff and Resident Quality of Life

Health and Safety

  • Support and supervise nursing staff, ensuring compliance with professional standards and regulatory requirements
  • Contribute to resident assessments, care planning, and interdisciplinary care conferences
  • Lead or assist in recruitment, onboarding, and performance evaluation of staff, particularly PSWs
  • Maintain accurate clinical documentation and support data submission requirements
  • Promote team unity, wellness, and quality of life for residents through effective communication and leadership
  • Act as a resource for best practices and policy updates, and champion ongoing staff education
  • Ensure efficient use of equipment and supplies, and foster community and academic partnerships
  • Provide regular updates to the Director of Care on quality improvement and program outcomes

Qualifications:

  • Registered Practical Nurse with current registration in good standing with the College of Nurses of Ontario
  • Minimum 3 years of RPN experience; management/supervisory experience preferred
  • Strong knowledge of dementia care, infection control, wound care, and adult learning principles
  • Experience in long-term care is an asset
  • COVID-19 vaccinations (2 doses) required to work in our facility
