Job summary

Services and Housing In the Province in Mississauga, Canada, is seeking a Community Mental Health Counsellor, Intake & Assessment. You will be the first point of contact for applicants and families, assess needs, and refer to the most appropriate programs while coordinating with community services.

In this role you will work with a multidisciplinary team, use standardized tools, build a community resource network, and ensure person- and family-centered care with PHIPA compliance and a commitment

Qualifications

• Post-secondary education in Social Work, Social Services, Health Services, or equivalent.
• 3+ years' experience providing assessments on individuals with mental health challenges.
• Knowledge of serious mental illness, concurrent disorders, and/or dual diagnosis; intake experience preferred.
• Excellent assessment, risk management, and crisis intervention.
• Expertise in collaboration with healthcare services.
• Knowledge of MS Suite (Word, Excel, Power Point and Outlook).
• A vehicle, valid driver's license, and adequate insurance coverage.
• Valid Certification in First Aid & CPR.
• Training in ASIST and CPI.

Responsibilities

• Work within a multidisciplinary team to support mental health recovery and care.
• Uphold PHIPA legislation and promote equity, diversity and inclusion.
• Ensure records are kept and statistics maintained per agency standards.
• Adhere to Health & Safety Program and Client Safety Framework.

Position Summary:

Reporting to the Program Manager, the Community Mental Health Counsellor, Intake & Assessment, will be responsible for being the first point of contact to applicants and their families as they are introduced to our organization and its services.

Their main goal is to ensure that applicants are assessed and matched to the most appropriate programs and referred to community services as required and depending on eligibility requirements

• Provide streamlined transitions with "warm transfers"

• Provide comprehensive, holistic, person and family-centered access to services
• Work independently and collaboratively within a multidisciplinary team to carry out mental health recovery and support
• Understand and be able to integrate a philosophy of person-centered care into daily responsibilities

Reporting Function

The CMHC, Intake & Assessment will report directly to the Program Manager. They will work with both internal and external partners and will interact with all levels within the organization.

They will be a support system, resource, and an advocator for the team and Company.

Working Conditions and Physical Requirements

• Hours are typically from Monday – Friday, 8:30am – 4:30pm but may vary depending on client needs
• 18 months contract
• Average amount of physical effort is required
• Able to sit/stand for prolonged periods of time
• Able to answer and communicate telephone calls and general computer use
• Able to lift up to 20lbs (product, documents, etc.)
• Vehicle travel to sites for visits/walk-throughs and open doors as required.
