Detailed Overview

In collaboration with a multidisciplinary team, provides clinical and liaison services through an Indigenous lens to clients identified as First Nations, Métis, and Inuit in community programs across the Fraser Salish region participating in the Early Psychosis Intervention (EPI) Program. Incorporates traditional Indigenous models of health and healing into clinical assessment, care planning, treatment, consultation, and follow-up, respecting cultural practices and protocols. Utilizes case management to assess mental, emotional, physical, and spiritual health, formulate care plans, implement care, and evaluate outcomes. Provides outreach for vulnerable clients facing access barriers. Shares knowledge with team members and offers mental health and substance use education to clients, families, and communities. Partners with clients, caregivers, community organizations, FNHA, Métis Nation BC, and other stakeholders. Participates in program planning, policy development, research, and evaluation to develop culturally safe services for Indigenous peoples. Reports clinically to the designated Clinical Lead as required.


Responsibilities
  1. Functions as a primary therapist and case manager, conducting assessments, providing therapy, managing cases, and utilizing crisis intervention and outreach as needed.
  2. Collaborates with Indigenous communities to develop culturally safe and accessible health services, including organizing engagement events.
  3. Facilitates client access to community services and rehabilitative programs, liaising with agencies, providing referrals, and ensuring continuity of care.
  4. Coordinates care between community services and hospitals, ensuring appropriate referrals and smooth discharge planning.
  5. Assesses client placements, arranges care in facilities, and evaluates cultural safety and appropriateness of placements.
  6. Provides support for clients requiring enhanced case management, monitoring mental health, substance use, social adjustment, and routine activities.
  7. Maintains client records, reports, and documentation in accordance with policies, including consent procedures.
  8. Supports early identification and engagement of first episode clients through community presence and relationships.
  9. Advocates for clients, providing information on appeal processes and assisting with documentation.
  10. Provides education on mental health, substance use, and available services to clients, families, and communities.
  11. Delivers health promotion activities to increase awareness and access to services.
  12. Shares culturally appropriate materials and participates in ongoing education and orientation activities.
  13. Engages in quality improvement, risk management, and participates in committees and advisory boards.
  14. Contributes to clinical research and program evaluation respecting OCAP principles.
  15. Performs other related duties as assigned.

Qualifications

Education, Training, and Experience

Master's Degree in a relevant health discipline (e.g., Clinical Counselling, Social Work). Eligible for registration with the relevant professional body. At least one year of recent experience working with Indigenous populations in community or clinical settings, or an equivalent combination of education and experience.

Valid B.C. Driver's license and access to a personal vehicle with appropriate insurance.

  • Cultural competence, humility, and trauma-informed care perspective.
  • Effective interpersonal and communication skills for engaging Indigenous clients and collaborating with teams.
  • Understanding of early psychosis, pharmacology, and community resources.
  • Ability to apply policies, standards, and protocols.
  • Clinical skills including assessment, counseling, case management, and crisis intervention.
  • Knowledge of concurrent disorders.
  • Organizational skills to manage caseloads independently.
  • Willingness to learn and adapt to new methodologies.
  • Effective communication in English, both verbal and written.
  • Ability to operate related equipment and perform physical duties.
