Position Overview
Experience
  • Four years related community experience with minimum of two years clinical coordination experience.
  • Experience working in an integrated health care setting is essential.
  • Experience working with vulnerable populations and people who are unhoused is essential.
Education (Degree/Diploma/Certificate)
  • An undergraduate degree in health or social services is required.

SPECIAL TRAINING:

  • Training or certification in a community health, primary care or acute care setting essential.
Certification/Licensure/Registration
  • Responsible for maintaining and providing proof of active registration with the appropriate registering body.
  • Valid Driver’s License and access to a vehicle required.

  • Skilled in the practice of change management.
  • Sound knowledge of and understanding of clinical evidence-based practice.
  • An understanding and sensitivity for clients who are at risk.
  • Knowledgeable in the fundamentals of Service Excellence, Customer Service and Cultural Safety.
  • General knowledge of regulations and scope of practice documents that guide nursing and other related disciplines.
  • Demonstrated ability to critically think, analyze, conceptualize and apply relevant knowledge of practice.
  • Ability to work under minimal direction.
  • Ability to facilitate professional continuing education requirements of staff.
  • Knowledge of budgets and financial reporting systems.
  • Embraces on-going learning and self-development.

This position requires a current satisfactory Criminal Records Check (including Vulnerable Sector Search), Child Abuse Registry Check and Adult Abuse Registry Check as conditions of employment. The successful candidate will be responsible for any service charges incurred. A security check is considered current if it was obtained no more than six (6) months prior to the start of employment.
