Job Description

In this valuable role, the Clinical Unregulated Care Provider Supervisor is accountable for supervising PSWs and HSWs who care for clients within the community. The Clinical UCP Supervisor will assess, implement, and evaluate a wide range of clinical care interventions to ensure the delivery of safe, quality, and ethical care.

What you’ll do:
  • Supervise and support PSWs and HSWs to ensure practices meet standards and policies
  • Respond to concerns from staff, patients, families, and team members
  • Lead through meetings, huddles, and one-on-ones
  • Initiate patient safety incident investigations and ensure timely action
  • Support clinical documentation and reporting requirements
  • Engage patients in problem solving, health teaching, and building rapport
  • Mentor staff in problem solving and education
  • Maintain knowledge and skills to provide safe, effective services
What you bring:
  • Current RN or RPN registration with the College of Nurses of Ontario
  • Minimum 5 years’ experience as a practicing RN or RPN
  • Strong patient health assessment skills
  • Understanding of community nursing principles
  • Leadership and team management experience
  • Knowledge of clinical documentation standards
  • Excellent organizational and interpersonal skills
  • Commitment to professional development
  • Critical thinking and problem-solving skills
  • Valid driver’s license and reliable transportation
  • Ability to provide a current Vulnerable Sector Screening (VSS)
  • CNA Certification as a Community Health Nurse (CCHN(C)) is a plus
