ParaMed in Canada seeks an experienced Clinical Unregulated Care Provider Supervisor to oversee PSWs and HSWs delivering community care. The role demands strong leadership, clinical assessment, and adherence to standards while supporting nursing teams and enhancing patient outcomes.

The position emphasizes collaboration, mentoring, and ongoing professional development, with a focus on safe, ethical home health services and effective problem solving across care teams.

Qualifications

• Current RPN registration with the College of Nurses of Ontario and entitled to practice without restrictions.
• Minimum of 5 years’ experience as a practicing Registered Practical Nurse
• Comprehensive patient health assessment skills are require
• Possess an understanding of the principles of community nursing practice
• Proven experience leading and managing a team
• Knowledge of clinical documentation standards and practice guidelines is required
• Excellent organizational, interpersonal, and teaching skills
• Must possess a valid driver’s license and access to reliable transportation
• Able to provide a current, clear Vulnerable Sector Screening (VSS)

Responsibilities

• Supervise and support PSW’s and HSW’s to ensure practices comply with all legislated standards of practice as well as company policies and procedures.
• Respond to and advise/resolve concerns from community staff, patients/families, and other team members.
• Provide ongoing leadership through regular huddles, face to face meetings and one-on-ones.
• Initiate patient safety incident investigations and documents to ensure timelines and action plans are met.
• Work collaboratively with nursing team members to support clinical documentation and submission requirements according to specific funder reporting guidelines.
• Engage patients in problem solving using a collaborative approach, offering health teaching, building rapport and trust to identify patient health care needs, strengths, capacities, and goals.
• Engage staff in problem solving, offering education, coaching, and mentoring in resolution of problems.
• Maintain the knowledge, skills, and attitudes to provide safe, efficient, effective service in all areas of practice by keeping abreast of changes in practice setting that affect scope of practice.
