Job summary

Family Medicine Associates in Grande Prairie is seeking a Social Worker/Mental Health Therapist to support physicians and help patients develop social functioning and resources. You may assess and provide psychotherapeutic interventions and coordinate patient services, including referrals to external supports.

The role requires working with patients with depression, anxiety, trauma, and related conditions, offering short-term counseling and collaboration with a multidisciplinary team.

Qualifications

• Registered Social Worker with ACSW, authorized to perform restricted psychosocial activities.
• Criminal Record and Vulnerable Sector check.
• Experience with providing therapeutic interventions to individuals or groups.
• Knowledge of local provincial mental health resources and community programs.

Responsibilities

• Provide clinical biopsychosocial assessments using various methods to assess mental health problems.
• Participate in early detection of emotional, behavioral and mental health concerns.
• Complete forms such as Disability Tax Credits and insurance forms as needed.
• Develop and implement clinical treatment plans and coordinate services.
