As an integral member of the department, the Clinical Records and Registration Clerk will represent the service in a professional manner, liaising with and supporting clinical teams, administration, patients, their families, and other healthcare community partners. The role involves matters related to patient registration, scheduling, and records maintenance. The Clerk will maintain and promote the confidentiality and security of personal health information at all times.


Skills And Qualifications Required


  • 2-year Health Information Management Diploma OR an equivalent combination of education and experience in a health records or registration setting.
  • One year of recent related experience in a hospital or medical setting, preferably in patient registration and/or scheduling, or formal education placement in a hospital clinical records department.
  • Experience working with electronic patient scheduling systems.
  • Certified Health Information Management professional preferred.

Position Responsibilities


  • Perform reception duties, including receiving calls, retrieving messages, returning calls, and transcribing messages to appropriate staff.
  • Serve as a front-line contact and receptionist for patients and customers.
  • Register new and returning patients in electronic systems and respond to client requests.
  • Schedule, maintain, and rebook appointments using electronic scheduling systems.
  • Manage Personal Health Information (PHI) records for admissions and registrations, in accordance with departmental procedures.
  • Monitor and track PHI record requests, outgoing and incoming.
  • Maintain the clinical record by filing, photocopying, and distributing reports.
  • Ensure PHI records are prepared, scanned, and indexed according to quality standards.
  • Import and extract records within electronic patient care records systems for electronic PHI management.
  • Maintain confidentiality and security of personal health information.
