Job summary

ParaMed is seeking a Clinical Planner to onboard new patients needing Personal Support Services in an assigned district. You will assess care needs, implement plans, and evaluate interventions within home health care across Canada, ensuring safe and compliant service delivery.

The role requires active RPN/LPN licensure, strong communication, and organizational skills to coordinate scheduling, maintain records, and protect patient privacy in a home setting.

Responsibilities

  • Perform initial assessment of care needs for new patients and develop individual care plans.
  • Conduct initial safety assessment of patient living environment to ensure safe working conditions.
  • Provide initial personal care services for new patients.
  • Advise Operations of scheduling parameters based on clinical needs.
  • Inform patients and families about ParaMed policies and scheduling practices and expected visit times.
  • Maintain current and accurate patient records.
  • Respect cultural needs, dignity, privacy, and confidentiality of patients and staff.
  • Stay updated on practice changes affecting scope of practice.
