Job Description
The Clinical Planner is accountable for onboarding new patients requiring Personal Support Services within an assigned district, through the assessment, implementation, and evaluation of a wide range of clinical and personal care interventions.

What You’ll Do
Perform the initial assessment of care needs for new patients and establishes the individual care plans for patient services.
Complete the initial safety assessment of a new patients’ living environment to ensure employees will be working in safe and healthy work conditions.
Perform the initial personal care services for new patients.
Advise Operations of the appropriate parameters for scheduling the patient’s ongoing visits, based on clinically indicated service requirements.
Inform patients and families of ParaMed policies and scheduling practices, as well as the expected time of care.
Maintain current and accurate patient records.
Show respect for cultural needs, dignity, privacy, and confidentiality of patients and staff.
Stay abreast of changes in practice setting that affect scope of practice.
What You Bring
Registration in good standing as a RPN/LPN with provincial governing bodies and minimum two years’ experience as a practicing nurse.
Working knowledge of applicable legislation including but not limited to Mental Health Act, Substitute Decisions Act, Healthcare Consent Act, Privacy / Personal Health Information Act, Occupational Health and Safety legislation and practices.
Knowledgeable in best practice standards for community health care and services.
Strong oral and written communication skills
Excellent organizational skills
Demonstrated competency in patient centered care principles and practices.
Knowledgeable of community resources available for patients and families.
Strong independent critical thinking and problem-solving skills.
Ability to provide a current, clear criminal back check with Vulnerable Position Screening (VPS)
