POSITION OVERVIEW

The Clinical Planner is responsible for onboarding new patients requiring Personal Support Services within an assigned district. This involves assessing, implementing, and evaluating a wide range of clinical and personal care interventions.


Accountabilities
  • Performs initial assessments of care needs for new patients and establishes individual care plans in accordance with Funder service requirements.
  • Completes initial safety assessments of new patients' living environments to ensure safe and healthy work conditions for employees.
  • Performs initial personal care services for new patients.
  • Assesses patient needs to determine the appropriateness for either Health Support Worker or Personal Support Worker care.
  • Assesses the possibility of shifting scheduled patient care to off-peak times based on clinical needs to increase system capacity.
  • Advises Operations on appropriate scheduling parameters for ongoing patient visits based on clinical requirements.
  • Informs patients and families about ParaMed policies, scheduling practices, and expected care times.
  • Maintains accurate and current patient records.
  • Respects cultural needs, dignity, privacy, and confidentiality of patients and staff.
  • Stays informed about changes in practice settings that affect scope of practice.
Qualifications And Experience
  • Registered in good standing as an RPN or LPN with provincial governing bodies and a minimum of two years’ experience as a practicing nurse.
  • Knowledge of applicable legislation, including the Mental Health Act, Substitute Decisions Act, Healthcare Consent Act, Privacy/Personal Health Information Act, and Occupational Health and Safety legislation.
  • Knowledgeable about best practice standards for community health care and services.
  • Strong oral and written communication skills.
  • Excellent organizational skills.
  • Demonstrated competency in patient-centered care principles and practices.
  • Familiar with community resources available for patients and families.
  • Strong critical thinking and problem-solving skills.
  • Ability to provide a current, clear criminal background check with Vulnerable Position Screening (VPS).
