The Clinical Planner is accountable for onboarding new patients requiring Personal Support Services within an assigned district, through the assessment, implementation, and evaluation of a wide range of clinical and personal care interventions.
Accountabilities
- Performs the initial assessment of care needs for new patients and establishes the individual care plans for patient services, in accordance with Funder service requirements.
- Completes the initial safety assessment of a new patients’ living environment to ensure employees will be working in safe and healthy work conditions.
- Performs the initial personal care services for new patients.
- Assesses patient needs to identify the appropriateness for either Health Support Worker or Personal Support Worker care.
- Assesses the ability to shift scheduled patient care to off-peak times, based on clinical need, in order to increase system capacity.
- Advises Operations of the appropriate parameters for scheduling the patient’s ongoing visits, based on clinically indicated service requirements.
- Informs patients and families of ParaMed policies and scheduling practices, as well as the expected time of care.
- Maintains current and accurate patient records.
- Shows respect for cultural needs, dignity, privacy, and confidentiality of patients and staff.
- Stays abreast of changes in practice setting that affect scope of practice.
Qualifications and Experience
- Registration in good standing as a RPN/LPN with provincial governing bodies and minimum two years’ experience as a practicing nurse.
- Working knowledge of applicable legislation including but not limited to Mental Health Act, Substitute Decisions Act, Healthcare Consent Act, Privacy / Personal Health Information Act, Occupational Health and Safety legislation and practices.
- Knowledgeable in best practice standards for community health care and services.
- Strong oral and written communication skills
- Excellent organizational skills
- Demonstrated competency in patient-centered care principles and practices.
- Knowledgeable of community resources available for patients and families.
- Strong independent critical thinking and problem-solving skills.
- Ability to provide a current, clear criminal background check with Vulnerable Position Screening (VPS).
