Oxford University Hospitals NHS Foundation Trust is seeking a Pharmacist to support prescribing policies and medicines management within a GP practice setting. The role involves collaboration with GP Leads and the Business Manager to develop strategies, conduct reviews, and resolve complex prescribing queries.

You will review secondary care requests, perform structured medication reviews, and provide guidance on cost-effective prescribing and budget considerations.

Formación

• Review, develop and implement prescribing policies and strategies for the practice.
• Conduct structured medication reviews and address complex regimens.
• Discuss medication issues with patients and clinicians as appropriate.
• Review secondary care requests and respond to inappropriate prescribing.
• Monitor prescribing issues and advise on cost effective prescribing and budgeting.

Responsabilidades

• Work with GP Leads and Business Manager to review prescribing policies.
• Design and implement audits related to prescribing targets and QOF/IIF.
• Respond to internal queries to amend prescriptions and re-authorise repeats.
• Involve in education and training for staff on prescribing practices.
• Provide medicines information updates and keep the intranet prescribing folder up to date.
