Job summary

Fraser Health Authority in British Columbia seeks a Full-Time Clinical Pharmacist for the Community Medication Management Program. The role combines virtual and in-person home health care for Surrey and North Delta clients, with travel to home settings and collaboration across an interdisciplinary team.

Qualifications

• Current registration as a Pharmacist with the College of Pharmacists of British Columbia.
• Completion of an accredited Hospital Pharmacy Residency Program or equivalent.

Responsibilities

• Consult with physicians and health care providers to develop and discuss therapeutic plans.
• Individually tailor client medication therapies based on charts, labs and assessments.
• Provide client medication counselling and education to patients and families.
• Lead medication safety and quality improvement initiatives.
• Participate in home visits and program team rounds.
