Job Description

Sinai Health is seeking a Clinical Nurse Specialist (CNS) for Geriatric Emergency Management (GEM) at our Mount Sinai Hospital campus.

Mount Sinai Hospital is part of a network supported by The Regional Geriatric Program of Toronto (RGP). As a CNS GEM, you will be a member of the Emergency Department (ED) leadership team, providing specialized clinical resources for assessing and treating high-risk older adults in the ED. Your responsibilities include screening, assessing, coordinating care, and collaborating with interdisciplinary teams and community supports to develop care plans.

In this role, you will:
  1. Complete assessments to identify symptoms, health conditions, functional, emotional, cognitive status, and home environment issues in older adults in the ED.
  2. Make care recommendations for frail seniors and assist in their implementation across hospital services, including specialized geriatric services.
  3. Coordinate comprehensive care plans in collaboration with the ED team, patients, their families, and other healthcare providers.
  4. Develop linkages with community services.
  5. Educate patients, families, and staff about the needs of older adults and specialized care in the ED.
  6. Innovate solutions to improve care transitions between ED, inpatient units, and community providers.
  7. Support the development of a senior-friendly healthcare environment in the ED.
  8. Participate in performance reporting, quality improvement initiatives, and system-wide projects.
  9. Engage with RGP activities and maintain links with other GEM nurses.
  10. Uphold the standards and philosophy of the Department of Nursing.
Job Requirements
Mandatory:
  • Master's degree in nursing from a CNO-accredited institution (or equivalent as specified).
  • Current registration with the College of Nurses of Ontario.
  • Minimum 3 years recent experience in Emergency or Acute Medical/Surgical nursing.
  • Expertise in Geriatric Nursing.
  • Membership in the Registered Nurses' Association of Ontario (RNAO).
Preferred:
  • Canadian Nurses Association (CNA) certification in Geriatrics.
Skills and Knowledge
  • Strong theoretical knowledge and advanced clinical skills in patient care.
  • Effective communication, interpersonal, and customer service skills.
  • Experience leading and facilitating change within multidisciplinary teams.
  • Excellent organizational and time management skills.
