Job summary

Baycrest Hospital in Toronto is seeking a Clinical Manager for Transitional Care (3W), After Hours Managers, and Staffing Office to provide clinical and operational leadership for a diverse portfolio.

You will lead a high-performing interprofessional team, focus on quality, safety, and patient-family centred care as older adults transition from inpatient care to home or next level.

The role requires after-hours leadership, occasional evenings, weekends, and a temporary 12-month term.

Qualifications

• Membership in good standing with a regulated health discipline college in Ontario.
• Bachelor’s degree and a Master’s degree completed.
• Minimum 3 years of leadership/management experience in a hospital.
• Current and valid Basic Cardiac Life Support (BCLS) certification.
• Strong financial management skills.
• Experience in gerontology, post-acute care is an asset.
• Familiarity with patient flow, transitions to long-term care.
• Proven collaboration with system partners in healthcare.

Responsibilities

• Foster leadership of Transitional Care unit, After Hours Managers, Staffing Office.
• Support interprofessional team in delivering patient/family centred care.
• Implement quality documentation practices and accountability models.
• Promote an interprofessional approach to care.
• Contribute to mission, vision, values, and strategic plan.
• Develop and implement quality improvement processes with metrics.
• Build relationships with system partners.
• Provide care resources to ensure safe patient care.
• Lead to improve practice patterns and processes.
• Work with leadership on budgets and utilization targets.
