The Dorothy Ley Hospice offers both community and residential care to people living with a life-limiting illness or loss. Our goal is to enable individuals to live their lives to the fullest in comfort, with dignity, meaning, and hope. Our services, offered at no cost to the individual or their family members, address their physical, emotional, spiritual, and practical needs. Last year, Hospice staff and its more than 300 trained volunteers served over 2,500 individuals.

The Hospice has launched a new five-year strategic plan to build on the organization’s successes to expand hospice services to meet the growing demand for hospice care.

The Opportunity:

Dorothy Ley Hospice is seeking a compassionate and experienced professional to join our team in the role of Clinical Lead, Supportive Care and Bereavement. This leadership position plays a vital role in advancing the Hospice’s commitment to person- and family-centred care, ensuring that clients, residents, caregivers, and bereaved individuals receive holistic, respectful, and culturally responsive support.

Reporting to the Director of Community Programs and Quality, working collaboratively within an interdisciplinary team, this role provides clinical leadership for the development, implementation, supervision, and evaluation of the Hospice’s Supportive Care, Spiritual Care, and Bereavement Services. This role is integral to maintaining high standards of clinical excellence, fostering a supportive team environment, and building meaningful connections within the community.

Key Responsibilities:

Clinical Supervision and Team Support

The Clinical Lead plays a central role in fostering a supportive, resilient, and high-performing interdisciplinary team. This includes providing clinical supervision, promoting team wellness, and ensuring a consistent standard of care across Supportive Care, Spiritual Care, and Bereavement Services.

  • Provide ongoing clinical supervision to approximately regulated professions (RSW, RPs, and RN) in the Community Team members, including Care Coordinators, Bereavement Counsellors and Spiritual Care Coordinators. Support staff in maintaining professional standards, ethical practice, and emotional resilience.
  • Facilitate team huddles and allocation of cases and workload. Encourage open communication to build a shared understanding of the team’s collective objectives.
  • Provide oversight, guidance, and clinical input for challenging or high-risk cases. Act responsively to requests for assistance and guidance.
  • Provide direct care or case coverage during staff absences or in complex situations requiring additional leadership involvement, including conducting home visits when needed.
  • Facilitate case debriefs and reflective practice individually and as a group to promote shared learning, ethical reflection, and continuous growth.
  • Support the emotional well-being and resilience of staff by creating safe spaces for staff to explore the emotional and psychological impact of hospice work. Monitor signs of compassion fatigue, burnout, and vicarious trauma, and implement strategies to promote self-care, team cohesion, and psychological safety.
  • Act as a clinical resource and escalation point for urgent, complex, or ethically challenging situations, offering in-the-moment guidance and follow-up support to ensure continuity and safety of care.
  • Supervise student placements as needed
  • Promote a culture of respect, accountability, mutual support, and interprofessional collaboration.

Clinical Leadership and Quality Improvement

  • Bridge Clinical and Community-Based Models: Balance clinical best practices with a community-informed approach to service delivery. Foster partnerships that respect the social, spiritual, and cultural dimensions of care.
  • Ensures program policies, processes, and procedures are current and up to date to meet legislative, professional practice standards, accreditation standard,s and college requirements
  • Conduct regular chart reviews and audits to maintain clinical quality, identify gaps, and support continuous improvement.
  • Ensure the use of evidence-based tools for assessment, care planning, documentation, and client education.
  • Gather and analyze program data to inform decision-making, performance monitoring, and reporting to funders and donors.
  • Participate in ongoing quality improvement initiatives to advance service excellence and innovation.
  • Promote Equity and Access to Care: Actively work to remove barriers to service, ensuring that programming is welcoming, inclusive, and accessible to individuals from equity-deserving, underserved, and marginalized communities.

Community Hospice Volunteers

  • Champion the Integral Role of Hospice Volunteers: Recognize, support, and integrate the essential contributions of hospice volunteers into service delivery. Collaborate with the Volunteer Management staff to ensure volunteers are prepared, valued, and effectively engaged across Supportive and Bereavement Care programs.

Program Development and Evaluation

  • With the Director, Community Programs and Quality, participate in the development, implementation, and evaluation of programs in supportive care, spiritual care, and bereavement to meet the diverse and evolving needs of individuals and families served by the Hospice.
  • With the Director, Community Programs and Quality, design and deliver new initiatives that respond to community needs, service gaps, or emerging best practices in end-of-life and grief support.
  • With the Director, Community Programs and Quality, foster Grassroots and Community-Led Partnerships. Collaborate with grassroots organizations, community leaders, and local cultural, faith-based, and social service groups to enhance community access to hospice care.
  • Provides oversight for the planning, implementation, monitoring, and evaluation of Supportive Care and Bereavement Services in accordance with:
  • Client-focused expectations and outcomes;
  • Continuous quality improvement;
  • Organizational and funder service requirements and targets;
  • Provincial and accreditation standards, as appropriate ;and,
  • Organizational effectiveness and appropriate resource allocation.

Community Engagement and Partnerships

  • Represent Dorothy Ley Hospice in the development and maintenance of strong, collaborative relationships with external organizations, including healthcare partners, community agencies, academic institutions, and community agencies.
  • Build and nurture community partnerships that support inclusive, equitable, and accessible care for all clients, especially those from underserved or marginalized populations.
  • Support student integration through academic partnerships, ensuring an enriching and structured placement experience.
  • Participate in community outreach, staff appreciation, volunteer recognition, and fundraising initiatives.
  • Advocate for inclusive, community-driven hospice care through meaningful engagement and external collaboration.

Qualifications and Experience

  • Registered professional with a Master’s degree in social work or a related health care field.
  • A minimum of 5 years’ experience in a similar role
  • Depth of experience in at least one of: community hospice, grief and bereavement counselling, and/or spiritual care.
  • Recent supervisory and leadership experience
  • Knowledge of Hospice Palliative Care is required
  • Demonstrated knowledge and commitment to the principles of person- and family-centered care
  • Demonstrated experience in program planning, development, facilitation, and evaluation
  • Demonstrated knowledge of effective group facilitation and adult education principles is an asset
  • Recent experience providing training, education, mentorship and/or coaching others
  • Experience working with diverse communities with sound knowledge of the larger system and social service network, and demonstrated ability to form community partnerships
  • Experience with quality assurance initiatives; experience with accreditation process, preferred
  • Demonstrated experience leading change management initiatives
  • Demonstrated relationship management skills with the Shared Purpose, Vision, and Values of Hospice
