The Clinical Lead is an integral member of the TimminsAcademic Family Health Team (TAFHT) and supports the team in deliveringcomprehensive community health planning and promoting health initiatives.Reporting to the Clinical Director, the Clinical Lead provides oversight ofclinical supervision to designated Interdisciplinary Health Providers (IHPs).Key responsibilities include developing, coordinating, implementing,monitoring, and evaluating TAFHT programs and services. The Clinical Leadcollaborates closely with primary care providers, IHPs, individuals, communitypartners, and groups to ensure the effective implementation of health promotioninitiatives and strategies aimed at influencing the health behaviors of TAFHTpatients and the community.

PrimaryResponsibilities:

§ Conduct needs assessments to identify gapsin service delivery, assess and make recommendations.

§ Implements clinical plans and evaluationfor each program area that are aligned with the overall TAFHT goals andstrategic plan.

§ Conduct individual and group needsassessments for health promotion and disease prevention.

§ Develop effectivehealth communication resources for individual or group use.

§ Utilize social mediaand TAFHT website to promote resources, services and awareness.

§ Provide clinical supervision to IHPs related to the deliveryof TAFHT programs & services.

§ Assist with case consultations and review.

§ Manage theimplementation of practice standards for IHPs.

§ Monitor patient safetyand quality issues.

§ Demonstrate abilitiesin program implementation and the evaluation of outcome measures.

§ Prepare statistical reports and communicatesquality indicators, KPIs, ensures quality assurance, quality improvement, andevaluation activities for IHPs.

§ Educate and support theprivacy officer in delivery of training and prevention.

§ Knowledge and proficiencyin current, evidenced-based methods and practices of primary care delivery,with an emphasis on health promotion and disease prevention.

§ Ensures IHP coverage and assists with nursing coverage when necessary.

§ Provide and/orfacilitate training and education sessions to IHPs.

Education,Qualification and Skills

§ Current registration in good standing with theCollege of Nurses of Ontario as a Registered Nurse.

§ 2-3 years of clinical experience in primarycare, community health, or acute care setting, with a focus on working withvulnerable populations.

§ Strong knowledge of chronic disease management,mental health, and addiction services.

§ Excellent communication and interpersonalskills, with a strong commitment to patient advocacy.

§ Ability to work independently andcollaboratively as part of an interdisciplinary team.

§ Proficiency in the use of computers andinformation technology, including experience with electronic medical records(EMR), Practice Solutions preferred.

§ Experience working in community-based settingsor with underserved populations is an asset.

§ Ability to prioritize and adapt in a fast-pacedand constantly changing work environment.

§ Ability to speak and write in both officiallanguages, English and French is an asset.
