Hours: 37.5 hours per week
Salary: Commensurate with experience and consistent with UHN Compensation Policy
Status: Temporary Full-Time (approx. 15 months)

UHN’s Transitional Care Program (TCP) includes UHN’s Buttonwood Reactivation Care Centre (RCC), Hillcrest Reactivation Centre & Queens Estate Transitional Care units. The RCC aims to improve restoration to help patients maintain social, cognitive and physical functioning by leveraging patient-specific, goal-based service planning. Patients serviced by the TCP do not require acute care but need to engage in activities to improve their function and independence, provided less active medical management and more therapy and activity. TCP also serves several health system purposes, including:

  • Reducing ALC pressures on acute care hospitals, reducing the need for Long-Term Care Home placement, and
  • Decreasing the risk of re-admission to an acute care hospital.

Position Summary

Reporting to the Manager of Professional Practice, the Clinical Educator is a member of the TCP’s leadership team and works within the Collaborative Academic Practice portfolio. The Clinical Educator will be expected to possess a set of core competencies fundamental to developing and advancing best practices at the point-of-care across the three TCP sites. Key components of the role involve focusing on: staff education and development; promoting and enhancing interprofessional collaboration; facilitating knowledge translation into practice; as well as leadership and change management. The Clinical Educator must be proficient in assessing staff learning needs, and be able to develop, deliver and evaluate educational programs. The Clinical Educator will also collaborate with unit managers, other educators and stakeholders to support various practice-based or clinical care projects. Finally, the Clinical Educator will contribute to scholarship, a culture of inquiry and research, as appropriate.

Minimum Requirements:

  • Must be registered with the College of Nurses of Ontario and able to practice without restrictions.
  • Theoretical and practical experience in adult education including learning needs assessment, educational program planning, development, and evaluation.
  • Previous experience as an Educator preferred.
  • Three (3) to Five (5) years recent Nursing experience.
  • Previous experience in Level 2 Step Down unit environment.
  • Completion of specialty-specific certification program(s) relevant to respective clinical areas.
  • Current certificate of licensure/registration with the College of Nurses of Ontario.
  • Completion of a Basic Cardiac Life Support (B.C.L.S.) program, Level II: Basic Rescuer certification.
  • Excellent leadership, time management, and presentation skills.
  • Ability to be adaptable, build trust, provide coaching and collaboration, facilitate change and promote continuous improvement.
  • Demonstrate ability to motivate staff and create an environment conducive to learning and team building.
  • Sound knowledge base in areas of specialty and the education process.
  • Able to work effectively as a member of an interdisciplinary team.
  • Competence in use of information technology.
  • Vaccines (COVID-19 and others) are a requirement of the job unless you have an exemption on a medical ground pursuant to the Ontario Human Rights Code.
