Job description

Reporting to the Manager of Allied Health, the successful candidate will be responsible for assessing and implementing evidence-based nutrition interventions in an acute care environment. Collaborating within an interdisciplinary care team, the candidate will employ interventions according to the standards set by the College of Dietitians of Ontario and internal policies. The role may involve working in various settings, including inpatient surgical, medical, intensive care, stroke, geriatric rehab, high-intensity rehab, and outpatient clinics.

Duties
  1. Screen and identify patients requiring intervention using established nutrition care standards.
  2. Respond to and prioritize high-risk nutrition referrals from physicians, team members, families, and patients.
  3. Assess patients' nutritional needs using scientific evidence, physical assessment, medication interactions, biochemistry, medical history, and chart review.
  4. Develop, order, coordinate, and monitor high-risk nutritional care plans.
  5. Educate patients, families, and team members.
  6. Document all nutrition care accurately in medical charts.
  7. Contribute to hospital policies and procedures related to nutritional care.
  8. Provide education to team members, acting as a primary resource for evidence-based nutrition information.
  9. Coordinate with community groups and institutions to ensure seamless patient care transfer.
  10. Serve as a role model demonstrating interpersonal skills and commitment to professional growth.
  11. Stay informed on best practices, research, accreditation, policies, and legislative requirements, implementing improvements as needed.
  12. Mentor dietetic interns and students.
  13. Perform other duties as assigned.
Qualifications
  1. Registered with the College of Dietitians of Ontario in good standing.
  2. Experience with enteral and parenteral nutrition.
  3. Experience with medical directives.
  4. Ability to work effectively in a team within a fast-paced, stressful environment.
  5. Excellent customer service, interpersonal, and communication skills.
  6. Computer literacy in Microsoft and adaptability to different software environments.
  7. Ability to maintain effective interdisciplinary relationships for positive patient outcomes.
  8. Strong organizational and time management skills.
  9. Professional demeanor, empathy, and understanding of human and organizational behavior.
  10. Excellent reading, writing, and communication skills.
  11. Skill in translating scientific and medical information into practical treatment plans.
  12. Ability to identify nutritional disease states and prescribe appropriate therapy.
  13. Develop innovative methods to manage complex nutritional needs.
  14. Align with Hospital Mission, Values, and Standards of Performance.
