Program Overview
The Clinical Counsellor is a member of the Community Care Unit (CCU) reporting to the Supervisor, Youth Outreach Team. This role is integral to a holistic, collaborative team working within frameworks of anti-oppression, anti-racism, trauma-informed care, harm reduction, disability justice, and mental health recovery. Responsibilities include assessment, therapeutic counseling, clinical consultation, crisis intervention, and service linkage, requiring excellent clinical, organizational, and administrative skills.
What You Will Do
Direct Client Support
- Serve as an access point for youth with complex mental health issues.
- Perform psychosocial and psychological assessments to determine service needs and formulate treatment plans.
- Develop client-centered goals considering individual values, preferences, and circumstances.
- Provide therapeutic counseling using modalities such as CBT, Solution Focused Therapy, DBT, Motivational Interviewing, etc.
- Promote active communication among clients, caregivers, and healthcare providers.
- Coordinate with interprofessional team and community agencies for seamless care and referrals.
- Navigate hospital and community resources to support client care.
- Conduct risk assessments in crisis situations, devising intervention plans and providing crisis support.
- Identify and connect clients to appropriate resources, considering cultural and health literacy factors.
- Lead or facilitate support groups, developing necessary materials.
- Maintain accurate and timely client records.
Interprofessional Team Member
- Collaborate with the care team to facilitate client transitions across services.
- Participate in case conferences and clinical meetings.
- Enhance communication pathways between WoodGreen and hospital services.
- Collect data for research and service evaluation.
- Contribute to service development and improvement initiatives.
General
- Adhere to organizational policies and confidentiality standards.
- Mentor and supervise students and volunteers.
- Participate in team meetings, training, supervision, and performance reviews.
- Maintain documentation and statistical data for accountability.
- Represent the unit on internal/external committees as assigned.
- Perform additional duties as required.
Qualifications
- Masters of Social Work (MSW) or relevant graduate degree.
- Registered Social Worker (RSW) or Psychotherapist (RP) in good standing with OCSWSSW or CRPO.
- Minimum 3 years of related experience in child and youth mental health, addictions, or social services, supporting youth from an anti-oppressive, trauma-informed perspective.
Preferred Skills and Experience
- Experience supporting youth aged 16-29 with complex mental health and addiction issues.
- Knowledge of therapeutic modalities such as Narrative Therapy, CBT, DBT.
- Understanding of addiction theories and evidence-based treatments.
- Training in clinical interviewing and youth-focused interventions.
- Experience in crisis response, harm reduction, and interprofessional collaboration.
- Knowledge of community resources, social determinants of health, and systemic barriers.
- Strong assessment, de-escalation, and crisis intervention skills.
- Excellent organizational, documentation, and communication skills.
- Ability to work independently and as part of a team, demonstrating judgment and flexibility.
- Proficiency in MS Office and client information systems.
- Commitment to anti-oppression, client-centered, harm reduction, trauma-informed, and disability justice approaches.
- Fluency in English; additional languages such as French are assets.
