Program Overview – Counselling & Support Services, Community Care: The Counselling and Support Services team provides support for people with mental health and/or substance use issues, including concurrent disorders and dual diagnosis within an anti-racist, anti-oppressive, trauma-informed, harm reduction, disability rights, and mental health recovery frameworks.
Position: Clinical Counsellor (2025-288-CC), 12-month Contract Salary, Bargaining Unit (1 Vacancy)
The Clinical Counsellor provides assessment, case management, clinical and supportive counseling, service coordination, crisis intervention, advocacy, and outreach within the CSS team and other partner tables. The incumbent supports DE-OHT Stepped Care, MGH Child and Youth Anxiety Clinic, HATM, and Gerstein Crisis Centre by working with clients for short-term therapy.
What You Will Do
- Direct Client Service:
- Serve as a point of access to connect primary care providers to supports related to mental health and addictions.
- Establish contact with interprofessional team members and community agencies for seamless referrals.
- Navigate hospital and community resources to assist primary care providers in managing clients’ conditions.
- Perform clinical assessments to determine service needs and formulate treatment plans.
- Identify credible resources considering culture, language, accessibility, and health literacy.
- Support client-centered goal development considering clients’ values and circumstances.
- Provide system navigation, service/care coordination, and follow-up support.
- Offer short-term therapeutic counseling using appropriate approaches.
- Encourage active communication among clients, caregivers, and healthcare providers.
- Conduct preliminary risk assessments in crisis situations and provide crisis intervention.
- Create and update client records efficiently.
SCOPE Team Member
- Collaborate to improve client care during transitions within the healthcare system and community services.
- Enhance workflow and communication pathways between primary care and community agencies.
- Collect and submit data for research and evaluation.
- Contribute to service development and improvement.
General Program
- Implement organizational policies and maintain confidentiality.
- Mentor, train, and supervise students and volunteers.
- Participate in team meetings and ongoing training.
- Maintain documentation and statistical data.
- Represent the unit on internal/external committees.
Qualifications
- Masters of Social Work, Psychology, Counseling, or relevant postgraduate degree.
- Registration with Ontario College of Social Workers and Social Service Workers, College of Registered Psychotherapists of Ontario, or equivalent.
- 3+ years of experience in mental health, addictions, and/or social work.
- Experience supporting individuals with mental health and substance use issues from a harm reduction, trauma-informed, and recovery approach.
