Position Title: Clinical Case Coordinator

Program: Kinark Child & Family Services -> Syl Apps Youth Centre

Location: Reg/Temp - Temporary

Start Date: May 1, 2025

Temporary End Date: May 1, 2026

Full/Part Time: Full-Time

Total # of hours per week: 35

# of Positions: 1

Close Date: April 11, 2025

Job Description:
Identifying and coordinating internal and external resources for youth and their families during the youth’s admission, as well as their transition to the community; coordinating aspects of the referral process to Syl Apps Youth Centre; and providing some group and/or individual psychotherapy.

Duties & Responsibilities:

  • Case management to coordinate and support appropriate services for youth and their families while at Syl Apps.
  • Identifying, developing and coordinating appropriate discharge plans for youth by collaborating with internal and external stakeholders.
  • Verbal communication and writing documentation that addresses youth’s care and treatment. Coordinating meetings and appointments as mandated and/or as needed.
  • Facilitating skills-based groups and psychotherapy groups for youth.

Qualifications:

  • Requires completion of University degree in Social Work.
  • Registered Social Worker with the Ontario College of Social Workers and Social Service Workers.
  • Some practical or position-related experience required.
  • Satisfactory Criminal Record Check and Pre-Employment Medical.

** Please note that Equivalent combination of education & experience will be considered.
