You’ll work across departments and with community partners to promote efficient resource utilization and improve health outcomes. Key Responsibilities
  • Manage a panel of assigned members, guiding them along the continuum of care to enhance their quality of life and functional level.
  • Conduct thorough assessments of physical health, psychosocial, functional abilities, and social determinants of health.
  • Develop individualized care plans and connect members to necessary services and community supports.
  • Lead interdisciplinary care team meetings and care transitions to promote clear communication and coordinated care.
  • Educate members, caregivers, and volunteers on benefits, medications, and available resources.
Requirements

These are the qualifications typically needed to succeed in this position. However, you don’t need to meet every requirement to apply.

  • Education & Experience: Bachelor’s or Associate’s degree required. Two years of clinical experience and three years of managed care experience preferred. Valid California license as an RN, LCSW, LMFT required (PHN preferred). Unlicensed MSW/ASW candidates welcome to apply.
  • Knowledge: Familiarity with case management principles, Medicare and Medi-Cal programs, community resources, and HIPAA regulations.
  • Skills & Abilities: Strong communication, interpersonal, and collaboration skills. Ability to manage multiple priorities and work independently.
  • Other Requirements: Member-centric approach to care coordination and the ability to adapt to a dynamic healthcare environment.
