Job description

Closing the Gap Helathcare (CTG) is dedicated to providing Canadians with the highest level of care across a wide range of home-based and community-setting services.

We are hiring a Client Service Director, Integrated Care Solutions (ICS), to lead and oversee client operations across our Integrated Care Solutions Services. Reporting to the Vice President of Partnerships and Business Performance, the Client Service Director, ICS , is pivotal in driving business and operational development, fostering key external relationships, and ensuring the seamless delivery of our healthcare services across various geographic areas.

This is a full-time permanent position based in Mississauga. This role will support our GTA and northern communities in a hybrid home, in-branch, and in-office environments.

What you’ll be doing :

  • Lead the implementation of CTG’s Integrated Care Solutions Services with internal and external stakeholders
  • Responsible for providing strategic and operational leadership to home and community care programs in assigned geographic areas. Your goal will be to ensure a seamless experience of our integrated healthcare services for our clients, their families, caregivers, and healthcare providers.
  • Build, lead, and coach a team of high-performing Client Service Managers and Supervisors to develop a muti-professional workforce model with agility to work across the integrated healthcare system
  • Build and maintain relationships with primary care networks, Ontario Health Teams, acute care, LTC and community partners, provincial funders, and other key stakeholders to advance integrated healthcare services' thinking and approaches.
  • Engagement of clients, families, and providers in program development and co-design?
  • Create and implement business and growth strategies by analyzing and understanding market data and working to bring it from an idea on a page to an executable plan for change and improvements.
  • Develop actionable plans to meet budgetary responsibility and fiscal management for the assigned portfolio
  • Collaborate with other members of the Senior Governance Team to support the delivery of CTG’s corporate goals, analyze local data, and establish plans for continuous improvements to the quality of service for our clients.
  • Report on program performance and report KPIs to a broad audience of partners

Skills needed for success :

  • Minimum of 5 years in progressive leadership positions in healthcare or home and community environments
  • Minimum of 5 years of operational experience building new programs and sustaining operations; managing expenses in relation to revenue; analyzing the ongoing performance of the portfolio to ensure that established operational, clinical, and financial goals are being achieved; completing monthly and annual reports as requested.
  • Master’s degree in Business, Health Leadership, Health Informatics, or a health-related discipline.
  • Strong knowledge of business and clinical operations principles, practices, and methods, service delivery, financial management and control, program development, implementation, and evaluation.
  • Proven ability to build and lead high-performance teams
  • Experience in operational planning, development, and evaluation with the ability to identify long-term strategies and goals.
  • Excellent networking, influencing, and interpersonal skills, with the ability to work at all levels of organizations, internally and externally, exercising a good level of diplomacy
