The purpose of this position is to provide direct care to residents, under the supervision of a licensed nurse, in accordance with facility policies and procedures.
Qualifications
· State-required training/certification.
· Active registration on state Nurse Aide Registry.
Delegation of authority
Authority is delegated to the individual in this position to:
- Provide care to assist residents with their activities of daily living as well as other activities designated on the resident's plan of care or as assigned.
- Report resident needs and concerns to a licensed nurse.
This position reports to:
Charge Nurse
- Follow written and oral directions.
- Follow established performance standards and perform duties according to nursing service policies and procedures.
- Assist new employees in following established facility policies and procedures.
- Identify resident problems and concerns and report them immediately to a Charge Nurse or a licensed nurse.
- Complete assignments timely, completely and accurately.
- Conduct resident rounds as assigned.
- Has successfully completed an approved CNA program as required by state and/or federal regulations.
- Identify safety hazards and emergency situations and initiate corrective action.
- Participate in in-service classes as assigned and complete assignments.
- Assume personal responsibility for following facility procedures related to control of equipment and supplies within the unit.
- Report incidents and/or unusual problems according to established facility procedures. Report within shift it occurs.
- Participate in interdisciplinary team and other facility meetings as assigned.
- Participate in the development of an individualized plan of care of assigned residents.
- Review care plans and provide care as outlined.
- Record the care and treatment provided to the resident and the resident's response or lack of response to care provided.
- Maintain confidentiality of resident and facility information.
- Report changes in resident conditions to Charge Nurse or Nurse Supervisor.
- Follow facility procedures for admission, discharge and transfer of residents.
- Complete documentation accurately, timely, following facility policies and procedures.
- Listen to resident and family complaints and report to Charge Nurse.
- Ensure residents have call lights in reach. Answer call lights promptly.
- Assist with activities of daily living as designated in each resident's care plan.
- Participate in the planning and implementation of the individualized plan of care for assigned residents.
- Ensure residents are given adequate assistance with meals, nourishment and transportation to meal service area(s).
- Check name bands on all assigned residents daily and report to Charge Nurse any bands that have been removed or need to be changed.
- Consistently work cooperatively with residents, charge nurses, treatment nurses, restorative nurses, other nursing assistants, physicians, families, consultant personnel and ancillary service providers.
- Observe all facility safety policies and procedures.
- Observe all facility policies and procedures.
- Report for work in a clean, neat uniform and consistently present an appropriate professional appearance.
- Report for work as scheduled and consistently demonstrate dependability and punctuality.
- Assume accountability for data contained in the employee handbook.
- Perform other related duties as directed by his/her supervisor.
- Perform all duties assigned in an effective, timely and professional manner.
- Follow facility's infection control procedures.
- Follow Resident Rights policies at all times.
- Be responsible for the safety of residents under his/her supervision.
- Accept assigned duties in a cooperative manner.
- Complete annual 12 hours in-service programs as designated by federal regulations.
- Attend training in dementia management and resident abuse prevention as assigned.
- Participate in all compliance and ethics training programs.
- Use only authorized abbreviations established by the facility when recording information.
- Agree not to disclose assigned user ID and password for accessing resident/facility information. Promptly report suspected or known violations of such disclosure to the Charge Nurse or Compliance and Ethics Officer.
- Agree not to disclose resident's protected health information and promptly report suspected or known violations of such disclosure to the Charge Nurse or Compliance and Ethics Officer.
- Assist residents with bathing, oral care, dressing/undressing, toileting, perineal care, catheter care, grooming, eating and taking/recording vital signs in accordance with facility policies.
- Strip bedding, change linens and make resident beds as assigned.
- Ensure that department computer workstations left unattended are properly logged off.
- Report all accidents/incidents to your supervisor no matter how minor they may be. Report such occurrences on the shift in which they occur.
- Report any known or suspected unauthorized attempt to access facility's information system.
- Report suspected resident abuse immediately to the Administrator.
- Promptly report suspected violations of the facility's compliance and ethics program to the facility's Compliance and Ethics Officer.
