The Intake Nurse is responsible for facilitation of intake for Central Referral office services within IWK Health. This includes the utilization of appropriate screening tools, risk assessment, clinical triage and disposition, consultation, advice, and information sharing. This individual also facilitates referral support into and out of Children’s Health Program (CHP). The Intake Nurse demonstrates competencies in clinical leadership, guidance, and support, to determine the overall direction of care and initial coordination of services based on identified person-centered needs. The Intake Nurse demonstrates proficient knowledge, skills, and ability needed to provide competent, evidence-based care/service. The Intake Nurse contributes to the efficient and effective functioning of the service/unit. Accept responsibility for maintaining and continually enhancing competencies through ongoing professional development including participation in education programs, research, and continuous quality activities.
