Job description

Care Coordinator – Lifemark Health Group

Clinic Name and Location: Lifemark Physiotherapy Stony Plain, 4300 South Park Drive, Stony Plain, AB, T7Z 2W7.

Status: Permanent Full Time

The Care Coordinator is responsible for fostering a strong relationship between the clinic and its customers, including patients, staff, funders, and referral sources. This is achieved by efficiently providing administrative support and coordinating clinic activities with friendly, enthusiastic, and professional service to all patients, team members, and external customers.

The Care Coordinator handles all aspects of patient intake, data entry, answering phones, outbound calling, receiving faxes and emails, as well as invoicing and payments.

Primary Responsibilities

  1. Answer telephone calls, greet patients and visitors upon arrival, and provide excellent customer service for all questions or concerns.
  2. Coordinate detailed and accurate intake of information during the patient's first visit and throughout their treatment plan.
  3. Assist patients with completing paperwork at the clinic to ensure accuracy.
  4. Schedule patient appointments and maintain medical records.
  5. Collect payment at the time of service.
  6. Track coverage for all funding streams to ensure approval.
  7. Complete data entry and billing.
  8. Coordinate courier packages (incoming and outgoing) and process emails, mail, and faxes.
  9. Provide administrative support to the Care Coordinator Manager and Clinic Director.

Qualifications & Core Competencies

  1. High School Diploma, G.E.D., or equivalent.
  2. Excellent customer service skills and telephone etiquette.
  3. Strong organizational skills with the ability to prioritize and multitask.
  4. Excellent verbal and written communication skills and interpersonal skills.
  5. Proficiency in computer use and data entry.
  6. Flexibility and adaptability to change.
  7. Experience in a physiotherapy clinic is an asset.
