Fraser Health Authority in White Rock, BC is seeking a Casual Psychiatric Liaison Nurse to join the Emergency Department team. The role focuses on intake assessment, crisis intervention and collaboration with interprofessional care teams to support patients with psychiatric and concurrent disorders.

Frontline duties include triage of referrals, care planning, crisis counselling, and ensuring smooth patient flow through discharge planning and community referrals.

Formación

• Graduation from an approved school of Nursing or Psychiatric Nursing.
• Current practicing registration with the British Columbia College of Nurses and Midwives (BCCNM).
• One year recent related experience in acute or community mental health.

Responsabilidades

• Screens psychiatric referrals from a variety of internal and external sources; identifies service priorities and triages for psychiatry.
• Performs psychosocial assessments and collects collateral related to the patient’s supports and environment.
• Develops care plans with final dispositions authorized by the physician and provides crisis counselling as needed.
• May provide short-term follow-up and case management services for adults in the Emergency Department.
• Communicates with interprofessional teams and coordinates discharge planning and linkages to community resources.
• Documents assessments, care plans, interventions and referrals according to Fraser Health standards.
