This is a casual on-call LPN position with opportunities to transition to a permanent role. The successful candidate will work alongside the Assisted Living and Long-Term Care Program Manager and Care Aids to provide care to adults at our Assisted Living and Long-Term Care locations in Burnaby and Surrey. The role may involve supervision, administrative tasks, delegation, and supporting caregivers who assist individuals who are physically disabled with minor mental health issues and/or have acquired brain injuries. The successful candidate should be highly organized and possess strong leadership skills.

Locations include:

  • Liberty Place, 7899 17th Avenue, Burnaby, BC V3N 1M1, CAN
  • Freedom Place, 10342 148th Street, Surrey, BC V3R 3X3, CAN
Responsibilities
  • Providing clinical care, support, and assistance to individuals with disabilities
  • Developing and implementing health care plans
  • Completing daily documentation
  • Administration of medication
  • Working as a team
  • Identify and assign permissible tasks within scope of practice
  • Maintain documentation and reports (e.g., Log Books, Communication Books, Professional Visitation Forms, Quarterly Reports, Care/Service Plans, Individual Program Plans, Reportable Incidents)
  • Collaborate with Program Manager to develop tenant care plans and service plans upon entry and as needs arise
  • Update tenants’ progress based on care plans every three months
  • Provide Direct Support Services as well as Administrative support
Qualifications
  • LPN Registry number in good standing and current certificate
  • Minimum of 1-year experience
