ROLE PURPOSE

This position will provide outreach services to clients who are experiencing mental health issues and/or developmental disabilities. Mobilize community resources to ensure that clients are provided with support, assistance, life skills assistance, and information.

STATUS AND CLASSIFICATION
Union. This position is open to applicants of all genders.

SHIFT HOURS

Rotations as follows:

REPORTING AND WORKING RELATIONSHIPS

  • Reports to the Manager of Community Living Services or their designate
  • Function as a multi-disciplinary team member
  • Liaises with staff and volunteers of community and government agencies as required

KEY ACCOUNTABILITIES

  • Identify the needs of individuals referred by the agency's government partners at admission
  • Ensure adequate supervision of clients in the community and Connective's housing facilities
  • Develop care plans with the Case Management Team addressing clients' psycho-social-emotional needs
  • Conduct group or individual counselling sessions using basic counselling techniques
  • Monitor each client's progress and develop strategies to address their care plan
  • Conduct program audits and evaluations as scheduled
  • Meet with clients within 24 hours of referral
  • Conduct client evaluations within the first 30 days and adjust contact frequency as needed
  • Establish a network of community resources for each client
  • Assist clients in sustaining levels of clinical and behavioural functioning
  • Monitor medication regimes as directed by a psychiatrist
  • Provide crisis intervention as required
  • Ensure clients maintain contact with community agencies
  • Facilitate programming for life skills development
  • Access community programming to develop a support network
  • Provide skill building in areas such as parenting and self-management techniques
  • Participate in activities to stimulate appropriate socialization behaviours
  • Identify options and make referrals for safe and affordable housing
  • Involve clients' family and friends for support
  • Utilize agency systems and technology platforms effectively
  • Perform other related duties as required

WORKING CONDITIONS

  • Function independently while being a crucial member of the support team
  • Adhere to Connective’s policies and procedures
  • Work flexible hours during daytime, evenings, and weekends based on operational needs

QUALIFICATIONS

  • Two (2) years related experience working with clients experiencing mental health issues
  • Counselling experience with clients interacting with the criminal justice system

REQUIREMENTS

  • Criminal Records Checks
  • Emergency First Aid
  • Class 5 driver’s license and access to a reliable vehicle
  • Proof of vaccination for communicable diseases may be required

COMPENSATION

The wage range for this position is $27.54 to $31.61 per hour as per collective agreement.

Important: Candidates must be legally authorized to work in Canada and provide proof of eligibility if selected for the role.
