Job description

SLCS is currently seeking a Full-Time Case Worker for the Supported Independent Living (SIL) Program. The people we support live within the community and receive assistance with connection to resources, life skills education and activities of daily living. As a member of this interdisciplinary team, you will provide support services and crisis intervention to adults with disabilities, mental health needs and/or addictions. Case workers may support people who have the potential to become verbally or physically aggressive in a one to one setting.Responsibilities of this position include:

  • Facilitate development of individual case plans with adults with developmental disabilities and at risk of or currently involved in criminal justice system who require support to live independently and reducing recidivism
  • Connect client to necessary resources in order to achieve their current goals
  • Educate clients on various life skills to facilitate their independence
  • Assess risk and need of clients in relation to community safety and care plans
  • Liaise with other team members to coordinate care for clients.
  • Provide education, support, and supervision to other staff or students as applicable
  • Regularly travel between SLCS facilities and community locations
  • Establish and maintain professional working relationships with community organizations and other agencies
  • Ensure all documentation and paperwork is completed according to standards of care
  • Connect with client’s family and supports to remain engaged and assist as a resource for wellbeing of client

This position does not include personal support type work such as personal care or medication administration.

The hours of the program run between 8am - 10pm, 7 days a week. The successful candidate will be required to work a rotation of 12 hour shifts as well as one 8 hour shift every two weeks on a Saturday. The rotation includes a 3 day weekend every other weekend.Required Qualifications
  • University degree or 2- 3-year College diploma from an accredited institution
  • 1-3 years’ work experience dealing with people with developmental disabilities, acquired brain injuries, addictions services, and/or fetal alcohol spectrum
  • 1-3 years’ experience working with people who have the potential to become verbally or physically aggressive, using de-escalation strategies to mitigate risk.
  • Standard First Aid / CPR - Level C
  • Valid G2 or G Class Driver’s License and access to a reliable vehicle
  • Case management experience is an asset
  • Candidates who are proficient in both official languages of Canada will be considered an asset
