Job Description:
Responsibilities:
The incumbent is responsible for the following range of job duties characteristic, or typical of the job class:
- Monitors the environment and clients/residents to ensure safety and security; completes regular client/resident status checks and room searches as required.
- Develops and implements case management of assigned clients/residents; assists clients/residents in establishing goals, monitoring progress, and prepares discharge plan.
- Maintains and updates all appropriate documentation (including in-out and incident log books) related to clients/residents and initiates any new documentation; complete all reports required by the funder and The Salvation Army.
- Provides safe storage and recording of prescribed medications.
- Coordinates and facilitates resident programming as required.
- Conducts all required shift change procedures prior to and after shift change.
- Performs intake and discharge procedures for clients/residents as assigned.
- Monitors appropriate behaviour; intervenes in crisis situations, provides emotional support and advocacy as required.
- Engages in the development of inter-personal relationships that promote dignity and respect.
- Performs required scheduled check-ins of residents on passes in community; escorts residents in the community when necessary and monitors behavior.
- Liaises with Police, funders, court personnel, community agencies, families, and other staff members.
- Provides orientation to newly hired staff, volunteers, and students.
- Provides back-up support to front line position and their functions as required.
- Responds to day to day “urgent” needs of clients.
- Attends and facilitates case conferences with case management team.
- Collects and records all case management statistics on a daily basis.
- Prepares monthly outcome statistical report for Director.
- Prepares monthly outcome measures report for Director.
- Reviews and forwards weekly resident update reports to the Region of Halton.
- Reviews all resident intakes, discharges and restrictions.
- Prepares weekly Personal Needs Allowance requisitions.
- Conducts tours of building for groups or individuals as requested by the Director.
- Attends all seminars and training sessions as requested by Management.
- Performs other position related duties as assigned.

Organizational Responsibilities:
- Adhere to standards that are consistent with the values & philosophy of The Salvation Army and the Halton Lighthouse.
- Participate in all Case management and Staff Team Meetings.
- Be knowledgeable of Fire Safety & Emergency Evacuation Plans.
- Responsible to work safely and report all unsafe or unhealthy working conditions to the Health and Safety Committee.
- Engage in formal evaluation sessions with his/her supervisor at 3 and 6 months during the first year of employment and annually thereafter.
- Read and follow the Policy & Procedures Manual and the Employees Handbook.
- Every employee must read (and sign that they have read) a copy of the Residential Policies and Procedures, and understand that they will be required to adhere to all of these Guidelines in their dealings with Clients, as well as with Staff and Administration.
- Make suggestions that will improve efficiency, working conditions or procedures to Management.
- Represent the Organization in a professional manner.

Qualifications:
- Completion of more than two (2) years of Community College.
- Post-Secondary Education or equivalent with a background in Social Work, i.e. Social Services or Human Services Diploma preferred.
- A minimum of one (1) year of prior related experience, including front-line casework experience in a social services environment.
- Solid background of how to work with people from diverse backgrounds and enjoy assisting others to solve problems.
- Strong oral and written communication skills.
- Self motivated and disciplined.
- Knowledge of issues facing persons experiencing housing crisis.
- Knowledge and sound understanding of the Mental Health and Addictions support systems available to individuals in the region of Halton.
- Knowledge of various social issues pertaining to homelessness and its impact on the families served.
- Good interpersonal skills.
- Ability to work independently as well as part of a team.
- Strong networking ability to develop healthy partnerships with new and existing groups and organizations within the region of Halton.
- Certified in First Aid/CPR and Non-Violent Crisis Intervention.
- Valid Ontario Driver’s License, personal vehicle required and insurance, current copy of driver’s abstract that is satisfactory to The Salvation Army, in its sole discretion, is required.
- Experience/Working knowledge of Microsoft Office.
- Provide an original copy of a Criminal Background Check with vulnerable sector screening that is satisfactory to The Salvation Army, in its sole discretion, is required. The screening is secured either through the national Canadian Police Information Centre (CPIC) or through a local police detachment.
- Screening through The Salvation Army Abuse Registry.
- An understanding of the mission and purpose of The Salvation Army in Canada.
- NOTE: Alternative combinations of education and experience may be considered.
PHYSICAL EFFORT/DEXTERITY INVOLVED:
The performance of the job requires a moderate amount of keyboarding/manual record keeping, and occasionally lifting/packing clients/resident’s property/personal effects and engaging in recreational activities.
CONCENTRATION REQUIRED:
The performance of the job requires attentive listening and observation of body language, continuously monitoring the environment, multitasking, and reading.
RESPONSIBILITY FOR FINANCIAL RESOURCES:
The incumbent is involved in minor financial matters regularly, or on a relief basis, or has responsibility for the wise use of his/her own resources. He/She may be authorized to spend, disburse, or collect small amounts of cash. The incumbent has very little input into the annual budget.

RESPONSIBILITY FOR HUMAN RESOURCES:
The incumbent is responsible for his/her own time and effort only. He/She may demonstrate work methods to new employees, volunteers and/or students.
WORK ENVIRONMENT:
The incumbent’s work environment is typically in an office at a facility setting.
The incumbent will work under the following disagreeable conditions:
- Isolation
- Dealing with angry people and verbal abuse
- Exposure to bodily fluids and bodily waste
- Exposure to drugs and alcohol
EXPOSURE TO HAZARDS:
The job incumbent may be exposed to the risk of minor to moderate injury or illnesses due to:
- Physical attacks by others
- Exposure to infectious diseases
- Exposure to toxic chemicals
- Exposure to bodily fluids
- Sharp objects
- Other potentially dangerous situations
PRECAUTIONARY MEASURES:
The incumbent must take the following precautionary measures (in addition to health and safety rules):
- Universal Precautions
- Current Certification in First Aid/CPR
- Non-violent crisis intervention training
Compensation:
The target hiring range for this position is $20.66 to $21.12 with a maximum of $21.57. Placement in the salary range will be based on factors such as market conditions, internal equity, and candidate experience, skills, and qualifications relevant to the role.
