Job summary

Sault Area Hospital in Sault Ste. Marie, Ontario is seeking a Case Reviewer to work with the CRUM team and interdisciplinary care teams to optimize pre-admission through discharge planning. You will evaluate plans, coordinate care, identify barriers, and facilitate timely discharges.

You will lead patient/family conferences, monitor day triggers, support community partnerships, and contribute to quality improvement while maintaining current nursing knowledge and compliance with standards.

Qualifications

• Current registration with the College of Nurses of Ontario in good standing.
• Current BLS certification.
• Minimum 5 years recent clinical experience in a hospital inpatient setting.
• Experience as a Case Reviewer and in discharge planning.

Responsibilities

• Participate and collaborate with other health care providers in the provision of patient care.
• Effectively use the Standards of Nursing Practice and the nursing process to assess, plan, implement, and evaluate patient care.
• Coordinate and lead patient/family care conferences in collaboration with the inter-professional team.
• Engage with community partners to support and create supportive and safe plans.
• Participate in daily bullet rounds identifying readiness for discharge barriers.
• Maintain documentation meeting College of Nursing practice standards.
