The Case Manager provides holistic support to youth through individualized case management, action planning, and life skills development. This role involves facilitating educational workshops both online and in person, while fostering strong connections with schools, community agencies, and support networks to enhance program impact. The Case Manager ensures accurate documentation, data collection, and adherence to program guidelines and funder requirements. Responsibilities also include maintaining a safe and welcoming environment, coordinating peer mentorship opportunities, and managing program resources effectively. The position requires a commitment to continuous collaboration and excellence in service delivery.
Toronto Wagner Green YMCA Centre is a part of YMCA Employment and Community Programs which provide a variety of employment and counselling programs to youth ages 14-24 throughout the GTA. It provides a variety of supports to homeless and street-involved youth in the Queen Street West area. Services include employment, housing, substance use, harm reduction, mental health and other related supports. The Wagner Green YMCA is a 2SLGBTQIA+ positive space where all sexual orientations and gender diversity are welcome and celebrated. These services and supports are provided directly by the YMCA of Greater Toronto and community partners.
The Wagner Green YMCA Youth Pre-Employment Program is a specialized program for homeless and at-risk youth. This program includes life skills coaching, job search supports, action plan creation, and one-to-one employment supports with follow-up.
In this role, you will:
- Deliver and facilitate educational webinars and workshops online and in person to youth.
- Work one-on-one with youth to provide holistic support in developing life skills and aspirations through a case management process.
- Ensure a safe, comfortable, and welcoming environment for participants.
- Collect information to confirm eligibility and complete participant intake and/or assessment to inform ongoing support and achieve participant and funder outcomes.
- Design, implement, and evaluate youth-centered individual action plans, in-person/virtual services, and workshops in accordance with guidelines and funder mandates.
- Perform case management in alignment with guidelines and funder mandates.
- Consult with social workers, school administration, other professionals, and family/natural supports when appropriate.
- Coordinate peer mentorship opportunities and relationships.
- Maintain knowledge of relevant services and community resources.
- Engage with schools, community agencies, and other organizations to maximize services and resources available to participants.
- Adhere to all Risk Management and Occupational Health and Safety requirements.
- Input data accurately and in a timely manner into program-designated database systems.
- Work within defined program budgets.
- Maintain accurate participant files in accordance with funder and YMCA guidelines.
- Collect demographic, output, and outcome data for evaluation purposes and to support the development of funder and YMCA reports.
- Travel and stay overnight as required.
- Perform other duties as assigned.
You bring:
- Post-secondary education in Child and Youth Care Counselling, Education, Social Work, Recreation Management, or related field or equivalent experience.
- 2 years of experience working directly with vulnerable youth.
- 2 years of experience in client intake and assessment, case management, and workshop facilitation.
- Experience working with diverse populations.
- Experience within a team environment and working remotely.
- Working knowledge computer skills (Windows, Microsoft Teams, Office, and Outlook).
- Standard First Aid & CPR-C and able to obtain a Police Information Check and Vulnerable Sector.
- Working knowledge of issues/practices related to the social determinants of health, youth development, individual assessment, case management, group work, group facilitation, program planning, and outcome evaluation.
- Working knowledge in culturally sensitive and trauma-informed practice, and strength-based approach.
- Non-Violent Crisis Intervention, Motivational Interviewing, Suicide Intervention Certification preferred.
- Excellent organizational and time management skills.
- Excellent communication, interpersonal, and team skills.
- Valid driver’s license and access to a vehicle, an asset.
- Excellent understanding of diversity, inclusion, and equity including the ability to identify and address barriers, work effectively with diverse stakeholders, and integrate equity into program design.
