Candidates can be based in Ontario, but must be able to work either 11am to 7pm, or 12pm to 9pm, EST hours.

What You Will Be Doing
  1. Act as the single point of contact for patients enrolled in the support program and prescribing physician.
  2. Conducts welcome calls to gather relevant information, introduce patients to the support program, and obtain patient consent as required.
  3. Informs the patient about all program offerings e.g., case management support, reimbursement support, self-injection training, prescription coordination and ongoing compliance.
  4. Maintain an expert level of knowledge of the Canadian reimbursement landscape and current trends.
  5. Liaise with patients, insurers and physicians regarding the documentation required to maximize reimbursement coverage, including the exploration of all public and private coverage as needed.
  6. Complete and review insurance-related paperwork prior to submitting it to insurers for processing. Follow up on, and advocate for patients with public and private payers until a decision is received and coverage is secured.
  7. Complete financial assessments with patients in accordance with Program offerings and ensure financial assistance renewals are done in a timely manner.
  8. Ensure that reimbursement renewals are completed promptly for all assigned patient files.
  9. Coordinate directly with Specialty Rx Pharmacies and Specialty Health Clinics to ensure patients have access to and receive their treatment promptly.
  10. Assist Specialty Rx Pharmacies with billing as needed.
  11. Coordinate and maintain patient appointment schedules to ensure optimal clinic and homecare efficiencies are achieved. This includes scheduling initial patient appointments, rescheduling established series of appointments, and collaboration with nursing/clinics regarding staffing challenges.
  12. Contact patients to ensure adherence to the treatment plan and coordinate delivery of medication(s).
  13. Report all Adverse Events (AE) and Product Quality Complaints (PQC) immediately or as soon as possible upon becoming aware of the event as per client requirements.
  14. Maintain detailed case notes and ensure assigned patient files are accurate and up to date in the CRM database.
  15. Participate in and assist with continuous improvement initiatives related to processes, quality, training, etc. Adhering to determined KPIs.
  16. Act as a resource for Program stakeholders, which may include physicians, nurses, patients, clinics, and the client.
About Your Qualifications
  1. A bachelor’s degree in psychology, human services, healthcare, social work, or another related field is typically required.
  2. For senior or supervisory roles, a relevant master’s or doctorate degree may be preferred.
  3. Organizational and decision-making skills
