Summary
The Case Manager - Independent Living with Supports will provide leadership, development, and implementation of care plans for youth in the Independent Living Program. This role involves overseeing daily operations, managing finances for the caseload, and developing the program. The case manager will counsel youth on various issues to help them reach goals related to life skills, employment, and education.
PRIMARY RESPONSIBILITIES
- Provide case management for youth in the Independent Living with Supports Program.
- Manage monthly finances and expenditures for your caseload.
- Develop Risk Assessments, Safety Plans, and Care Plans with assistance from the program Occupational Therapist.
- Meet regularly with the Program Manager/Occupational Therapist for collaboration, planning, and oversight.
- Ensure a systems approach to planning.
- Coordinate collaborative, youth-directed discharge and transition planning.
- Supervise and mentor the Tenancy Support Mentor (TSM).
- Schedule support staff according to youth needs.
- Complete detailed and timely record keeping; ensure TSM does the same.
- Provide crisis management.
- Coordinate medical, therapy, and external appointments.
- Communicate with the placing agency, external collaterals, and other Link services as needed.
- Ensure participant homes/apartments meet residential tenancy standards and are equipped with necessary furnishings and essentials.
- Build relationships with landlords, including follow-up on complaints or evictions.
- Assist youth in searching for, viewing, and applying for apartments.
- Prepare leasing application packages and coordinate with management and the Link team.
- Complete move-in/move-out condition reports with management or building caretakers.
- Participate in staff meetings and contribute to agency functioning by assuming additional responsibilities as needed.
- Promote positive teamwork and role-modeling.
- Provide services through a holistic lens, focusing on youth strengths and experiences.
- Perform other duties as assigned.
Qualifications
Education:
- Post-secondary degree in Education or Human Services preferred.
Experience:
- Minimum of two years' experience working with high-risk youth.
- Proven ability to network and build community partnerships.
- Effective leadership skills.
- Understanding of the impact of colonization and the Calls to Action from the Truth and Reconciliation Commission.
- Excellent organizational, interpersonal, and communication skills.
- Proficiency in Microsoft Office (intermediate level).
- Knowledge of community resources, strengths-based, and solution-oriented approaches.
- Crisis intervention, harm reduction, and conflict resolution skills.
Working Conditions
- Flexible scheduling based on program and participant needs, including days, evenings, and weekends.
- Valid driver’s license, clean driver’s abstract, and access to a vehicle (mileage paid).
- Willingness to obtain required training (CPR/First Aid, De-Escalation, etc.).
- Clear criminal and vulnerable sector checks within the last 3 months.
Additional Details
Salary starts at $24/hour, with benefits, paid sick leave, vacation, and pension. The position is for one (1) role, working 37.5 hours/week, including some evenings and weekends.
