Job summary

The YMCA of Greater Toronto in Brampton is seeking a Case Manager to support youth referred by Probation with employment readiness, anger management, family counselling, and drug-use counselling. You will develop individual action plans and provide one-on-one counselling to help youth reach short- and long-term goals.

Qualifications

  • Post-secondary education or related experience in youth services or social work.
  • Minimum 2 years working with vulnerable youth.
  • Experience in intake, assessment, case management, and workshop facilitation.
  • Experience with diverse populations and remote teamwork.
  • Strong computer skills (Windows, Teams, Office, Outlook).
  • Able to obtain Police Information Check and Vulnerable Sector clearance.
  • Knowledge of social determinants of health and youth development.
  • Trauma-informed and equity-focused practice; preferred certifications.

The program will assist youth referred by Probation for a variety of interventions, including employment, anger management, family counselling, and drug use counselling. The Case Manager creates an action plan with youth referrals to ensure both short and long term goals are met. The Case Manager provides and prepares the youth for the identified interventions through one to one counselling, referral to community resources and consultation with external organizations to ensure successful outcomes. The Case Manager works with, and is part of program delivery, within a fast-paced employment centre offering multiple program opportunities for adult and youth clients.

In this role, you will:

  • Deliver and facilitate educational webinars and workshops online and in person to youth
  • Work one-on-one with youth to provide holistic support in developing life skills and aspirations through a case management process
  • Ensure a safe, comfortable, and welcoming environment for participants
  • Collect information to confirm eligibility and complete participant intake and/or assessment to inform ongoing support and achieve participant and funder outcomes
  • Design, implement, and evaluate youth-centered individual action plans, in-person/virtual services, and workshops in accordance with guidelines and funder mandates
  • Perform case management in alignment with guidelines and funder mandates
  • Consult with social workers, school administration, other professionals, and family/natural supports when appropriate
  • Coordinate peer mentorship opportunities and relationships
  • Maintain knowledge of relevant services and community resources
  • Engage with schools, community agencies, and other organizations to maximize services and resources available to participants
  • Adhere to all Risk Management and Occupational Health and Safety requirements
  • Input data accurately and in a timely manner into program-designated database systems
  • Work within defined program budgets
  • Maintain accurate participant files in accordance with funder and YMCA guidelines
  • Collect demographic, output, and outcome data for evaluation purposes and to support the development of funder and YMCA reports
  • Travel and stay overnight as required
  • Perform other duties as assigned

You bring:

  • Post-secondary education in Child and Youth Care Counselling, Education, Social Work, Recreation Management, or related field or equivalent experience
  • 2 years of experience working directly with vulnerable youth
  • 2 years of experience in client intake and assessment, case management, and workshop facilitation
  • Experience working with diverse populations
  • Experience within a team environment and working remotely
  • Working knowledge computer skills (Windows, Microsoft Teams, Office, and Outlook)
  • Valid Standard First Aid and CPR Level C certification OR Intermediate First Aid and CPR level C certification issued by a WSIB approved provider prior to the first day of work
  • Able to obtain a Police Information Check and Vulnerable Sector
  • Working knowledge of issues/practices related to the social determinants of health, youth development, individual assessment, case management, group work, group facilitation, program planning, and outcome evaluation
  • Working knowledge in culturally sensitive and trauma-informed practice, and strength-based approach
  • Non-Violent Crisis Intervention, Motivational Interviewing, Suicide Intervention Certification preferred
  • Excellent organizational and time management skills
  • Excellent communication, interpersonal, and team skills
  • Valid driver’s license and access to a vehicle, an asset
  • Excellent understanding of diversity, inclusion, and equity including the ability to identify and address barriers, work effectively with diverse stakeholders, and integrate equity into program design
  • Focus on the health, safety and wellbeing of all children, youth, and vulnerable people in YMCA programs including understanding legal and moral obligations related to reporting suspected child abuse to Child Protection Services
