The Case Manager will conduct outreach to Promise Healthcare patients identified with severe and/or chronic health needs, and/or further screening or assessment. Provide outreach to patients identified and referred by Promise Healthcare providers or external partner organizations. Will link clients to appropriate services, advocating on their behalf, monitoring effectiveness of treatment and providing services that support access to care.
ESSENTIAL DUTIES & RESPONSIBILITIES:
- Assist in continuous development and enhancement of a process for external patient referrals from partner organizations.
- Assist in continued enhancement of a process for internal referrals for Promise Healthcare patients identified through screenings as needing further assessment or services.
- Conduct outreach to patients and community members identified as in need of services and assist them in accessing services. This includes assisting with appointment scheduling.
- Work with Promise Clinical staff and enabling staff including Care Coordinators, Nurses, Providers and others involved in delivery of care in addressing identified Social Determinants of Health including transportation, food insecurity, lack of support and other social determinants.
- Work with Promise Referral Coordinators to help patients access health care services.
- Work with Promise Enrollment staff to help patients and community members enroll in Medicaid, Marketplace products and Promise Sliding Fee Scale assistance.
- Reach out to all new patients, meet with them and serve as an ambassador to connect them to services.
- Assess patient feedback and work collaboratively to address barriers to care that patients face.
- Provide educational information to patients about health center services and help connect them to all services.
- Work towards meeting targets for grants and quality programs.
- May assist with community outreach activities and events to promote access to services.
MINIMUM QUALIFICATIONS:
- COVID19 vaccinations and influenza vaccinations are required.
- Bilingual in Spanish and English strongly preferred.
- Bachelor degree in social work or related field.
EDUCATION / EXPERIENCE REQUIREMENTS:
- Experience at an FQHC (Federally Qualified Health Center) is helpful.
- Prefer experience working in community work. This can include advocacy, volunteering, philanthropy, etc.
- High School Graduation or Equivalent required.
- Bachelor degree in social work or related field preferred.
- Customer service experience is a plus.
