Direct Service - Regulated - (OPSEU)

Full-Time, Contract (Approximately 12 months)

Multi-Site Accountability; Oxford Catchment Area (Oxford Zone)

An Urgent Case Management Clinician works within a multi-disciplinary team framework throughout Oxford County, providing rapid response and support to individuals presenting with urgent, complex barriers and needs, including those with a serious and persistent Mental Illness, Developmental Disorder, Acquired Brain Injury, and/or Concurrent Disorders. The Urgent Needs Clinician will work closely with the CMHA crisis team and community partners on the Oxford County Situation Table.

This program offers support for a time-limited duration, and is brief and transitional in nature, focused on timely intervention and goal-oriented engagement.

Successful candidates must have a Police Vulnerable Sector Check (PVSC) completed within the last six months. Applicants needing a PVSC are encouraged to apply for one early to avoid delays.

WHAT WE'RE OFFERING:

  • Starting rate is $30.01 per hour (Level 1 of the Direct Service, Regulated Professional Grid). Successful external applicants will start at Level 1, with potential to progress to Level 8, $37.26 per hour.
  • Work schedule: 70 hours over a two-week period, Monday-Friday 8:30am-4:30pm.
  • Benefits: 9% in lieu of health benefits.
  • Pension: Eligible for immediate, optional enrolment in the Healthcare of Ontario Pension Plan (HOOPP).

HOW YOU WILL MAKE A DIFFERENCE:

  • Utilize the bio-psychosocial model and best practices for co-occurring disorders.
  • Protect clients' welfare, dignity, and rights.
  • Use Motivational Interviewing to support clients.
  • Maintain safe work practices and professional conduct.
  • Have knowledge of community resources.
  • Build therapeutic relationships with clients.
  • Work from a client-centered, trauma-informed perspective.

URGENT NEEDS (Assessment, Crisis Support, Follow-up):

  • Support a caseload of urgent needs clients.
  • Assess mental, physical, and emotional needs and investigate resources.
  • Engage in screening and assessments.
  • Provide skill development, linkages, resources, counseling, and advocacy.
  • Implement crisis prevention, intervention, and safety plans.
  • Educate about medications, symptoms, and addictions.
  • Collaborate with care providers and community initiatives.
  • Participate in community outreach, Mobile Health Outreach Bus, and Naloxone programs.
  • Attend the Oxford Situation Table to support at-risk community members.
  • Assist with health appointments and recovery plans.
  • Provide navigational support and work to reduce barriers to care.
  • Monitor symptoms, medication, and daily activities.
  • Deliver basic life skills training as needed.
  • Encourage client participation in community activities and promote self-efficacy.
  • Support individuals from diverse cultural backgrounds.
  • Provide coverage for team members as needed.

REQUIREMENTS:

  • Police Vulnerable Sector Check (PVSC) completed within the last six months.
  • Qualifications: RPN, RN, or Bachelor's/Master's in Occupational Therapy, Social Work, Psychology, or related field.
  • Current registration with relevant Ontario college.
  • Ability to attend work regularly.

PREFERRED QUALIFICATIONS:

  • Valid Driver’s License and access to a vehicle.
  • Experience in community-based settings, crisis intervention, mental health, addictions, homelessness, and regional services in Oxford County.
  • Knowledge of pharmaceuticals, legal system, and working with ABI or Developmental Disabilities.
  • First Aid & CPR certification.
  • Bilingual in both official languages is preferred.
