Job description

Position Overview

The Case Coordinator will determine program eligibility and assess the need for care at home or in a personal care home for the elderly or infirm adult. They will coordinate the delivery of a broad range of professional and non-professional services based on the Case Coordinator Guidelines. Acts as a liaison and maintains communication with agencies and facilities involved with the client group.

Position Duties and Responsibilities:
  1. Receive referrals and assess clients and families to determine care needs related to Home Care.
  2. Perform assessment and prioritize needs based on patterns.
  3. Establish appropriate discipline coordination.
  4. Develop a care plan for the Continuing Care Program, including client needs, objectives, service provision, and evaluation criteria.
  5. Implement and coordinate the care plan, which may include initiating medical rehabilitation and consultative services as necessary.
  6. Provide professional intervention through counseling, teaching, guidance, crisis intervention, etc.
  7. Maintain current case counts and ensure proper submission of statistics.
  8. Plan and organize work schedules and manage caseload demands effectively.
  9. Utilize consultation and supervision.
  10. Gather data on resources and community needs related to caseload.
  11. Interpret the program and resources provided through the Continuing Care Program to the public and other agencies.
  12. Establish and maintain liaison with local healthcare services and community resources.
  13. Participate in educational development for new staff, students, and related training.
  14. Stay current with developments in own discipline related to the Continuing Care Program.
Experience:
  • Two years of community experience preferred.
  • Previous experience as a Case Coordinator in Community Health Services preferred.
  • Experience with EFT implementation is an asset.
Education:
  • Bachelor's degree or related health/human service diploma/certificate (e.g., BSW, OT, PT, BSc PN, RPN, BN, RN, SLP, RD).
Certification and Licensure:
  • Valid driver’s license and vehicle.
  • Licensure/registration as per professional designation.
  • Registered Dietitian must have graduated from an accredited dietetic internship program.
Qualifications and Skills:
  • Computer knowledge and experience required.
  • Effective oral and written communication skills.

Additional Notes:

This term position may end earlier as outlined in the collective agreement. A current satisfactory Criminal Records Check (including Vulnerable Sector Search), Child Abuse Registry Check, and Adult Abuse Registry Check are required. The candidate will be responsible for any service charges. Checks are current if obtained within six months prior to employment start date.
