Job Description

In accordance with the Vision, Purpose, and Values, and strategic direction of Vancouver Island Health Authority (Island Health), patient and staff safety is a priority and shared responsibility. The position involves continuous quality and safety improvements.

Under the direction of the Manager, this role functions within an interdisciplinary team, providing care based on a Patient/Client centered approach, aligned with Island Health's vision and values, and in accordance with BCCNM standards of practice, including Indigenous Cultural Safety, Cultural Humility, and Anti-Racism principles. The Care Transitions Liaison Nurse assesses, plans, implements, and evaluates care for clients with various health needs transitioning between care settings. Responsibilities include assessments, creating care transition plans, coordinating and sharing information with clients, families, community, and other care services outside the region, along with health teaching, support, consultation, and referrals, all while functioning as part of a multidisciplinary team. The role emphasizes maintaining client independence and self-care, complying with nursing regulations.

For Indigenous Health roles, approximately 80% of activities are hospital-based; for Child, Youth, and Family roles, 100% are hospital-based.

Qualifications
Education, Training, and Experience

Registration with BC College of Nurses and Midwives as a practicing RN or RPN. Knowledge of medical/surgical/gerontological nursing and familiarity with Island Health's services is required.

  • Indigenous Health roles require: at least two years working with Indigenous populations in acute or community settings, knowledge of Indigenous health protocols, and a valid BC driver's license.
  • Child, Youth, and Family roles require: extensive knowledge of Pediatric/Neonatal populations, including NICU or PICU experience, and at least two years nursing in acute pediatric/neonatal care.

  • Culturally appropriate service delivery to Indigenous populations.
  • Competence in operating related equipment and computer programs including MS Office, telehealth, and database management.
  • Physical ability to perform job duties.
