Role: Care Team Coordinator
Position Type: Full-Time, Contract
Contract Duration: June 2025 - December 2026
Location: Toronto

Closing the Gap Healthcare (CTG) is dedicated to providing Canadians with the highest level of care across a wide range of home and community settings.

We are seeking a passionate and dedicated Care Team Coordinator to join our team. In this role, you will coordinate and schedule home and community visits, communicate with healthcare providers, and ensure seamless service delivery while providing empathetic support to clients and providers. Your administrative skills will help keep our operations organized and efficient.

What You Will Be Doing
  1. Assist and interact with clients, delivering exceptional customer service and support.
  2. Present information about CTG programs and services to clients to promote engagement and support sales efforts.
  3. Schedule visits and manage communication between clients and providers, ensuring timely and effective service.
  4. Assess customer requests accurately and efficiently, addressing them immediately or escalating as appropriate.
  5. Maintain records of all client interactions, schedules, and relevant documentation to ensure smooth workflow.
  6. Work closely with an experienced leadership team, benefiting from their guidance and direction as you grow in your role.
Skills Needed for Success
  1. Exceptional communication and interpersonal skills, with proficiency in English.
  2. A passion for customer service and a desire to grow within the healthcare industry.
  3. Commitment to maintaining confidentiality and privacy of client information at all times.
