Description

PRIMARY DUTIES

  1. Respond to patient referrals within established timelines (internal and external).
  2. Complete comprehensive care assessments promptly, using the tools, forms, and processes aligned with policy and procedure guidelines.
  3. Document all required information, including details pertinent to client status and care planning.
  4. Ensure patient and care provider safety by accurately assessing risks using provided tools.
  5. Develop care plans tailored to individual patient needs, ensuring care providers have the necessary knowledge and skills.
  6. Reassess patients at interim visits to confirm the continued appropriateness of the service plan.
  7. Investigate, resolve, and report all customer-related complaints.
  8. Promote service delivery excellence through onboarding, mentoring, coaching, and performance management of field staff, including conducting performance reviews.
  9. Ensure team members meet their key accountabilities and performance expectations.
  10. Participate in meeting or exceeding utilization, retention, and engagement goals for staff and care providers.
  11. Identify staff training needs and develop action plans to address gaps, minimize risks, and ensure service quality.
  12. Ensure all reporting meets standards and expectations of CBI Home Health/We Care, stakeholders, and regulatory bodies.
  13. Complete all documentation accurately and promptly, including client charting.
  14. Identify potential risks with internal stakeholders and implement mitigation strategies, especially concerning missed visits and clinical care.
  15. Respond promptly and comprehensively to incident reports.
  16. Enforce policies and procedures, ensuring they reflect best practices.
Qualifications
  1. Current registration with the College of Nurses of Ontario (CNO) as a Registered Nurse or Registered Practical Nurse.
  2. At least 2 years of supervisory experience and/or experience in home health service delivery.
  3. Ability to establish rapport with clients and care staff effectively.
  4. Valid Ontario driver’s license and access to a reliable vehicle.
  5. Ability to provide effective solutions that align with policies and uphold best practices.
  6. Self-motivated with the ability to work independently and as part of a team.
