Position: Care Practitioner at Woodlands Neurological Rehabilitation Centre, York
The role involves ensuring the safe operation of shifts, collaborating with the nurse in charge, engaging in care planning, managing medications, supporting the MDT with patient timetables, maintaining the environment, and participating in audits and changes.
Working Hours and Salary
- 36 hours per week
- Days, Nights, Weekends as per rota
- Salary: £14.93 per hour
Key Responsibilities
- Provide quality care promoting independence, dignity, privacy, and safety.
- Support qualified staff in assessment, planning, implementation, and evaluation of care without direct supervision.
- Lead holistic care delivery as a Rehabilitation Team leader.
- Communicate effectively within the team, with healthcare professionals, patients, families, and visitors.
- Maintain high standards in a safe, inclusive environment.
- Assess, evaluate, and report actions to registered practitioners or the multidisciplinary team.
- Supervise and delegate tasks to junior team members, assessing their skills and competencies.
- Undergo training for medication administration, storage, and ordering.
- Participate in supervisions and act as a preceptor for new staff.
- Assist in creating care plans, case reviews, and discharge notes tailored to patient needs.
Essential Qualifications
- NVQ 3/4, foundation degree, diploma level, or equivalent through experience and training.
Desirable Skills and Qualifications
- Experience in the specialty area.
- Grade C or above in GCSE Math and English or equivalent.
- Willingness to undertake training to teach in the lifelong learning sector.
