As a Care Coordinator, you will assess and determine patient care needs and eligibility, provide access and referrals to community services, and engage with patients, caregivers, and other healthcare practitioners.

What will you do? Under the general direction of the Manager, Home and Community Care, the Care Coordinator is responsible for managing a caseload of diverse patients with various health concerns. The Care Coordinator will assess patient needs via phone, develop, implement, and update care plans based on health and psychosocial needs, and accommodate patient and caregiver wishes where possible. They will also determine eligibility for Ontario Health atHome Champlain-approved services and refer patients to community resources.
As part of the Intake Care Team, the Care Coordinator will assess and develop individual care plans over the phone, utilizing a multidisciplinary team approach to optimize health outcomes. This is an in-office, non-visiting role where referrals are prioritized based on complexity and urgency, adhering to Ontario Health atHome Champlain guidelines.

A rotating shift schedule with eight- and twelve-hour shifts between 8 am and 8 pm, seven days a week, totaling 70 hours. The role offers a hybrid work model combining in-office and telework.
