As a Care Coordinator (Permanent, Full-Time,start date: Immediately; Location: North Bay), you will assess anddetermine patient care needs and eligibility, provide access and referrals tocommunity services, and engage with patients, caregivers and other health carepractitioners.

Whether you work in our office, in the community, or a health carefacility – you will play a lead role in providing connected, accessible,patient-centered care – and be supported by our collaborative team thatincludes over 9,000 regulated health care and other professionals.

What will you do?

· In collaboration with patients and their families, assess careneeds, determine eligibility for services, and develop individual care andservice plans

· Linkpatients with service providers

· Coordinate and monitor care plan delivery

· Establish a helping relationship with patients and their families

· Balance patient needs and choices with available resources,ensuring patients’ values and preferences are respected

What must youhave?

· Membership, in good standing, with the applicableregulatory body:

  • College ofNurses of Ontario
  • Collegeof Physiotherapists of Ontario
  • Collegeof Occupational Therapists of Ontario
  • OntarioCollege of Social Workers and Social Service Workers
  • Recentexperience in community health or a related field
  • Knowledgeof the health care delivery system and community resources
  • Excellentinterpersonal, communication, assessment, problem-solving anddecision-making skills
  • Effectivetime management, prioritization and organizational skills, with theability to work independently and co-operatively in a busymultidisciplinary environment
  • Establishedability to accurately complete required documentation, reports and forms
  • Avalid driver’s license and access to a reliable vehicle
  • Advanceoral and written proficiency in English and French is essential
  • We have amandatory COVID-19 vaccination policy. As a condition of employment, allemployees are required to submit proof of COVID-19 vaccination statusprior to start date.

What would give you the edge?

· Experience working with diversepatient groups, e.g., multicultural, homeless, palliative, acquired braininjury (ABI), mental health, geriatrics, pediatrics

· Casemanagement experience or recent related community experience

· Abilityto speak French or another second language
