Are you an experienced registered nurse, physiotherapist, occupational therapist, or social worker seeking a rewarding career that cares for others in a professional practice that cares for you? You’re looking in the right place.

As a Care Coordinator (Temporary, Full-Time, start date: June 2, 2025, to March 20, 2026; Location: Sudbury), you will assess and determine patient care needs and eligibility, provide access and referrals to community services, and engage with patients, caregivers, and other healthcare practitioners.

Whether you work in our office, in the community, or a healthcare facility – you will play a lead role in providing connected, accessible, patient-centered care – and be supported by our collaborative team that includes over 9,000 regulated healthcare and other professionals.

As a valued team member, your mission will be to help our patients be healthier at home, while benefiting from our supports for professional growth, personal wellness, and work-life balance.

What will you do?

  • Assess care needs, determine eligibility for services, and develop individual care and service plans in collaboration with patients and their families.
  • Link patients with service providers.
  • Coordinate and monitor care plan delivery.
  • Establish a helping relationship with patients and their families.
  • Balance patient needs and choices with available resources, respecting patients’ values and preferences.

What must you have?

  • Registration as a Nurse, Physiotherapist, Occupational Therapist, or Social Worker.
  • Member in good standing with the relevant College of Ontario.
  • Relevant experience working in a healthcare environment.
  • In-depth knowledge of home healthcare and community resources in Ontario Health atHome.
  • Case management, assessment, and interviewing skills.
  • Problem-solving, good judgment, and decision-making skills.
  • Ability to prioritize duties, manage multiple patients, and organize workload efficiently.
  • Ability to complete documentation, reports, forms, and recommendations accurately.
  • Strong interpersonal and collaboration skills.
  • Effective communication skills through various means.
  • Listening, observation, and facilitation skills.
  • Understanding of roles of other healthcare professionals in Ontario Health atHome.
  • Knowledge of funding agencies supporting patients.
  • Understanding of relevant legislation.
  • Knowledge of practice standards and guidelines by the College of Nurses of Ontario.
  • Proficiency with computer software and Microsoft Office.
  • Valid driver’s license and access to a vehicle.
  • Advanced proficiency in English (oral and written).
  • Mandatory COVID-19 vaccination proof prior to start date.

What would give you the edge?

  • Experience with diverse patient groups (multicultural, homeless, palliative, ABI, mental health, geriatrics, pediatrics).
  • Case management or recent community experience.
  • Ability to speak French or another second language.
